Case management resource kit
Homelessness services are funded to work within a strengths based, consumer focussed case management framework.
The documents below were written when homelessness services were funded through the Supported Accommodation Assistance Program. Despite the age of these documents, they still provide a useful overview of the case management approach within homelessness services.
Case management is defined as a collaborative, consumer focussed approach. It is aimed at empowering and effectively working with consumers to meet individual need.
The Kit outlines the eight core elements of case management practice in homelessness services: - Entry/screening
- Assessment
- Planning
- Direct service
- Coordination
- Monitoring and Review
- Exit planning, case closure and follow up
- Evaluation
The following documents describe each of these elements and identifies how they might be applied in different homelessness contexts. The final document is a case management resource guide for Indigenous homelessness services. The Council to Homeless Persons has also developed a series of good practice guides for homelessness workers on:

Case management resource kit Part 8: Example policy and procedure guide
The following document provides an example policies and procedures for homelessness services. Agency policy and procedures now need to be in line with requirements set out in the service's Funding and Service Agrement (FASA) with the Department of Health and Human Services (DHHS). The FASA lists the relevant legislation, guidelines and DHHS Standards that agencies must comply with.
